Thank you choosing American Foot & Ankle Center for your foot care needs.  For your convenience our New Patient forms are now available for download.  Please move your mouse cursor over the "Forms" pull down menu and click on each individual form to be downloaded.   It is necessary that you print all the New Patient Forms and complete them before your visit in order to expedite the check-in process.

Before you arrive, collect your medical records and imaging results, such as MRI films or x-rays. If you are not sure where these are, please contact your referring physician.

 Bring to your first appointment:  

• v  Completed new patient registration forms
• v  Driver’s license or other photo identification
• v  Insurance card or workers comp/auto claim #
• v  Medical records, imaging results (MRI films)
• v  A list of any medications you are taking
• v  Any co-pay per your insurance plan, unless prior arrangements are made.
